Patient Agreement

This Patient Agreement (“Agreement”) governs your use of direct-access laboratory testing and related healthcare services offered through the Healos platform, operated by Revelation Diagnostics, LLC d/b/a Healos (“Healos,” “we,” “us”). This Agreement supplements the Terms of Service and Privacy Policy.
1. Nature of Services
Healos facilitates direct-access laboratory testing by connecting you with an independent physician network that reviews and authorizes lab orders, and with CLIA-certified reference laboratories that analyze specimens. Healos is a technology platform and does not practice medicine, render medical advice, or establish a physician-patient relationship with you.
2. Informed Consent for Laboratory Testing
By placing a lab order through Healos, you acknowledge and agree:
- Lab tests ordered through Healos are authorized by a licensed physician in an independent physician network. The ordering physician reviews your order but does not establish an ongoing physician-patient relationship with you.
- Lab results are for informational purposes and do not constitute a medical diagnosis. Abnormal results should be discussed with a qualified healthcare provider.
- You are responsible for reviewing your results and following up with a healthcare provider for interpretation, diagnosis, and treatment.
- Certain test results may be reportable to public health authorities as required by law.
- Testing involves inherent risks including, but not limited to, false positives, false negatives, and results affected by interfering substances (e.g., biotin supplements).
3. HIPAA Authorization
You acknowledge that the physicians and reference laboratories used through Healos are likely “covered entities” as defined under the Health Insurance Portability and Accountability Act (“HIPAA”). By using our services, you authorize Healos (as a Business Associate of these covered entities) to receive, maintain, and transmit your protected health information (“PHI”) for the purposes described in our Notice of Privacy Practices.
4. Insurance and Payment
CHARGES FROM HEALOS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR. THIS PROHIBITION APPLIES WHETHER THE INSURANCE PROGRAM IS PRIVATE (SUCH AS AETNA, BLUE CROSS BLUE SHIELD, UNITED HEALTHCARE, ETC.) OR PUBLIC (SUCH AS MEDICARE OR MEDICAID OR ANY OTHER STATE OR FEDERALLY FUNDED HEALTH PLAN OR PROGRAM).
All lab tests and services are paid for directly by you at the time of order. By placing an order, you agree to pay the listed price and any applicable fees (interpretation fee, lab draw fee).
5. Age Requirement
You must be at least 18 years of age to use Healos lab testing services. By placing an order, you confirm that you meet this requirement.
6. State Restrictions
Direct-access laboratory testing may not be available in all states. Certain states, including New York, New Jersey, and Rhode Island, prohibit or restrict direct-access testing. You are responsible for ensuring that you are eligible to order tests based on your state of residence.
7. Electronic Signature
By clicking “I agree” or otherwise submitting your electronic acceptance of this Agreement, you acknowledge that your electronic signature has the same legal effect as a handwritten signature pursuant to the Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. § 7001 et seq.) and applicable state law. You consent to conducting this transaction electronically and to receiving all related documents in electronic form.
8. Refunds and Cancellations
Lab test orders may be refunded only if the specimen has not yet been collected by the reference laboratory. Once a specimen has been collected, the order is non-refundable. See our Refund Policy for details.
9. Limitation of Liability
TO THE MAXIMUM EXTENT PERMITTED BY LAW, HEALOS’S TOTAL LIABILITY FOR ANY CLAIM ARISING FROM OR RELATED TO YOUR USE OF LAB TESTING SERVICES SHALL NOT EXCEED THE AMOUNT YOU PAID FOR THE SPECIFIC TEST OR SERVICE GIVING RISE TO THE CLAIM.
10. Governing Law
This Agreement is governed by the laws of the State of Florida, without regard to conflict of law principles. Any disputes shall be resolved in the courts of Palm Beach County, Florida.
