7. Payment, Insurance, and Refund Policy

Fees for tests, memberships, and related services are displayed at checkout or in your account, as applicable. You authorize us and our payment processors to charge your selected payment method for amounts due.

CHARGES FROM HEALOS MAY NOT BE SUBMITTED FOR REIMBURSEMENT FROM ANY INSURANCE OR OTHER HEALTH CARE BENEFIT PROGRAM OR PAYOR, WHETHER PRIVATE OR PUBLIC (INCLUDING MEDICARE OR MEDICAID).

All lab tests and services are paid for directly by you. Healos does not bill insurance on your behalf and is not affiliated with any insurance provider.

Refunds, if any, are handled according to our Refund Policy or as required by law. If you have questions about a charge or refund, contact us using the information in Section 15.

Our collection and use of personal information is described in our Privacy Policy, which is incorporated into these Terms by reference.

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, HEALOS AND ITS AFFILIATES, OFFICERS, DIRECTORS, EMPLOYEES, AND AGENTS WILL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR PUNITIVE DAMAGES, OR ANY LOSS OF PROFITS, DATA, OR GOODWILL, ARISING OUT OF OR RELATED TO YOUR USE OF THE WEBSITE OR SERVICES, WHETHER BASED ON WARRANTY, CONTRACT, TORT, OR ANY OTHER LEGAL THEORY, EVEN IF ADVISED OF THE POSSIBILITY OF SUCH DAMAGES. OUR AGGREGATE LIABILITY FOR ANY CLAIM ARISING OUT OF THESE TERMS OR THE WEBSITE SHALL NOT EXCEED THE GREATER OF THE AMOUNT YOU PAID TO HEALOS FOR THE SERVICE GIVING RISE TO THE CLAIM IN THE TWELVE (12) MONTHS BEFORE THE CLAIM OR ONE HUNDRED U.S. DOLLARS ($100), EXCEPT WHERE PROHIBITED BY LAW.
